Final Outprocessing Appointment

What is Required at Your Final Out-processing Appointment?

***You MUST be scheduled and you MUST be in uniform for your final out-processing appointment (ABU/Flight Suit/Blues Recommended)***

Final out-processing briefings are held every Monday, Wednesday and Friday at 0900 in Bldg. 2106 (MPF), Room 426.

You must be scheduled by your relocation counselor to attend.  Please show up at your scheduled time. If you do not arrive in uniform or have not arrived by your scheduled time, your appointment will be rescheduled, your Port Call is subject to cancellation and your First Sergeant and Commander will be notified. Failure to provide all of the required items below may result in delay of your departure.


All members must bring the following documents to your appointment:


  • Virtual Out-Processing Checklist - All items must be completed and virtually signed off prior to your final out-processing appointment. Even if the item is not applicable, you must contact the office of primary responsibility to sign you off. NOTE: The Post Office and Pass & ID offices will physically stamp your checklist. Please bring a completed checklist(s) with all completed green checkmarks and stamps. (More than one checklist is acceptable).

  • Fitness Assessment Printout - For all assignments you must be current as of your Report No Later Than Date (RNLTD) plus 42 days. If your fitness test expires before that date, report to your Unit Fitness Program Manager to schedule testing prior to your departure.

  • Port of Call Itinerary - SATO will send your reservations and electronic tickets to you via email. If you have not received those two weeks prior to your departure please notify your relocation counselor.

  • Immunization & Security Clearance Letters

  • 2 Copies of your Orders (and any Amendments, if applicable)


  • ALL PPC Requirements and Documents (i.e. – Passport/VISA, Weapons Qualification, SABC/CBRNE, etc.)

Other Required Documents (If Applicable):

  • AF FORMS 4380/1466 (Dependent Medical Clearances)


  • AF FORM 5 - Flight Records (Mandatory for all personnel on flying status)

  • Medical/Dental Records - For members on PRP or Flight Status: It is your responsibility to obtain and bring your medical records with you to your final out-processing appointment. For all others: both the active duty member and their dependents' medical records will be forwarded via certified mail with a copy of your orders to your gaining unit.
