Step 1: Sponsor fills out the “Application to Renew ID Card” & attaches the form in Step 3
Step 2: Sponsor certifies the DD Form 1172-2 online:
a) Access the DMDC website (Retiree sponsors can create a DS log-on) via this link.
b) Click "Renew/Replace Family ID Cards" next to the dependent’s name
c) Certify the DD Form 1172-2 for the dependent renewing ID card
Step 3: Sponsor submits required documentation via DOD SAFE:
a) Picture of Dependent: Passport style, from shoulders up, white/off white background, full face forward, neutral facial expression or a natural smile, both eyes open, no hat or head covering unless authorized etc.
b) Photocopy of Two Forms of Identity Source Documents: Scan unexpired IDs with the EXACT same name, front and back if applicable. Your current dependent ID card can be used as one of the two required forms of ID. Only one form of ID is required for dependents 17 years and under.
c) If your current dependent ID Card was lost or stolen, please provide a Lost Letter. Download the appropriate template at MPF SharePoint Site - Self-Help Documents
d) Completed Application to Renew ID Card: from Step 1
e) Open DoD SAFE and submit the documents listed above with your pass phrase and claim code (if applicable) to 786 FSS/FSPS at email@example.com. (If DoD Safe is not working, please send an encrypted email to our org box with the requested documents).
Step 4: Pick up ID card at MPF Customer Service Office.
Once the ID card is created, you will receive an e-mail from a Customer Service representative. If you do not receive an e-mail within 5 working days of the date you submitted your file, please contact us to inquire on the status. Dependents are welcome to use MPF Appointments Plus or come in during our walk-in hours to pick up their IDs. Families may make a single appointment to pick up all dependent IDs created.
Please bring the following with you to retrieve your new Dependent ID(s):
1. Current ID card or Lost Letter (if applicable). Note: Old IDs are required to be turned in and will be terminated and destroyed.
2. Other ORIGINAL form(s) of identify used in Step 3b.
3. Dependents must be present to sign for the ID card and for visual verification. The sponsor is not required to be present for ID card pick-up.
